Po štúdiách v USA sa presťahoval do Indie, kde pracoval ako redaktor novín Herald. Hoci tam strávil len dva roky, indický mysticizmus silno ovplyvnil jeho literárnu tvorbu. V decembri 1882 vydal svoj prvý román, ktorý kombinoval anglo-indickú spoločnosť s orientálnym tajomstvom, a už nasledujúci rok predstavil úspešnú novelu Dr. Claudius. V polovici 80. rokov 19. storočia sa vrátil do Talianska a usadil v Sant'Agnello, kde kúpil Villu Renzi, dnes známa ako Villa Crawford. Postupne publikoval obľúbené diela a venoval sa aj talianskej histórii. Jeho beletria sa tešila veľkej popularite vďaka jeho rozprávačskému talentu a znalosti historických reálií. Medzi jeho najznámejšie dielo patrí román Corleone (1897), ktorý dramaticky zobrazuje vnútorný boj mladého kňaza. Pri písaní Pražskej čarodejnice (1891) objavil svoj talent na vyvolávanie zimomriavok. Hrôzostrašné poviedky, ktoré vytvoril na prelome storočia, sú súčasťou zlatého fondu hororovej literatúry a inšpirovali autorov ako H. P. Lovecraft a Stephen King.
