Freshly graduated and full of idealism, Dr. Andrew Manson begins his first position as an assistant doctor in a Welsh mining town. He quickly faces the harsh realities: the population is impoverished and uneducated, and among his colleagues, indifference, incompetence, and corruption are rampant. Manson fiercely fights for better patient care and evidence-based medicine, supported by the love of his life, the village schoolteacher Christine. However, after numerous setbacks, constant hostility, and failed fresh starts, his supply of idealism begins to dwindle. Moving to London opens up a new world, where he encounters colleagues adept at enriching themselves with minimal effort and wealthy (but not necessarily ill) patients, soon becoming a celebrated society doctor. A severe personal tragedy eventually reminds him of the lost ideals of his early days.
